This privacy notice explains what information the council needs in order to deliver the Careline and support service.

Who is responsible for your personal information?

Chesterfield Borough Council will usually be the data controller for the personal information collected.

We also provide some services on behalf of other authorities. In these cases, the data controller is the commissioning local authority and we are acting as a data processor on their behalf, often in collaboration with your housing provider/landlord. If you are unsure, please contact us and we will be able to advise you who the data controller for your personal data is.

How we collect your personal data

As part of the Careline and Support service, the council may collect information about you in a number of ways, including:

• information you have provided to us
• information and notes about you collected by our officers when they visit you
• if we provide your service on behalf of another local authority, we will access information about you from them, or from your housing provider/landlord
• information may be passed to us by another local authority, health services, your GP or another wellbeing service at your request

Purposes of processing your personal data

We need to process personal information about Careline and Support customers in order to deliver the service. This might include using the personal information provided to:

• offer tailored advice and reassurance
• send a response officer to assist you
• call for the emergency services or for medical assistance (and provide them with relevant health information about you if necessary)
• call family, friends or other agreed contacts
• contact your GP (and provide them with relevant health information about you)

Your information will also be used for statistical and reporting purposes to review and improve our service provision.

Categories of personal data and lawful basis

In order to provide this service, we may need to process some or all of the following categories of personal information:

• contact details
• age
• National Insurance number and benefits entitlement (so that we can advise on funding)
• financial details (to take payment for the service)

Under data protection legislation, processing of this data is allowed because it forms part of the performance of a contract.

We also need to process some or all of the following categories of sensitive personal information:

• health data
• ethnicity – used for equality monitoring only
• religion – used for equality monitoring only
• sexual orientation – used for equality monitoring only 

Under data protection legislation, processing of your sensitive personal information will be done because we have your explicit consent to do so. In an emergency, processing of your personal information may occur to protect your vital interests. 

Where the lawful basis for processing your data is ‘consent’, this consent can be withdrawn at any time using the contact details below. Withdrawing consent for the use of your health data could impact the effectiveness of the service we can provide to you.

Who we share your personal data with

In an emergency, we will share information that you have provided to us, or that we observe about you, with the emergency services. With your consent, we will also pass information directly to your GP.

With your consent, we will share some information with Derbyshire County Council to determine if you are eligible to receive funding for the service.

If we are not the data controller for the information we hold about you, we will share the information we collect with the data controller – usually the commissioning local authority.
