We can help improve the health of people that are at risk of developing a medical condition through exercise.

Our health referral scheme sessions take place at both the Queens Park Sports Centre and the Healthy Living Centre, with qualified staff to guide people on their journey and help them achieve their goals.

How to take part in the scheme

People that live in Derbyshire, or are registered with a GP surgery in the Derbyshire County Health Service area, can ask their health professional to refer them to the health referral scheme.

All GPs, practice nurses, physiotherapists, community mental health workers and other health professionals can refer you on to the scheme if that person meet the criteria.

How the scheme works

Once someone has been referred onto the scheme, they will be sent an appointment with one of our Active Life trainers. At this appointment they will explain the ins and outs of what is involved.
