Personal housing plans
A personal housing plan - PHP - is an assessment carried out after you apply to join our housing register.
It helps us to better understand your needs, so we can help you to find the right home and provide additional support if you need it.
Who are personal housing plans for?
Personal housing plans are for all new housing applicants. We also carry out PHPs for ongoing applications where there has been a significant change of circumstance, for example, an additional family member or deterioration in health.
When do we complete the personal housing plan?
We will get in touch with you to complete a personal housing plan when we have received and checked your housing application.
Will you visit me at home?
Yes, visits are carried out to applicants living within 20 miles of our Stonegravels office. We carry out visits to applicants whose home is their permanent residence. Applicants not living in permanent accommodation can be assessed over the telephone.
What are the benefits of having a personal housing plan?
It helps us to better understand your housing, health and support needs. This means that we can allocate the right priority banding for your housing application and provide the right support before and after you have moved into your new home.
For example, you might be applying for your first home and may need tenancy support or help to access benefits you are entitled to. We may refer you to other teams or signpost on to specialist agencies.
At the visit, we can also answer any questions you might have, advise about likely waiting times and discuss other housing options which might be available to you.
Who will complete the personal housing plan with me?
A member of the housing and health team will complete the personal housing plan with you.
Is a personal housing plan required?
No, a personal housing plan is not required, but it is advisable. It enables us to better understand your needs to help us to house you appropriately.
What happens to the information I provide?
Information is stored alongside your housing application in accordance with the advice given in the privacy notices on our website and the Home Options website:
